Terms of use

The product

AI form creation is mobile-only; this site hosts public form links.

Credits and generation

• Credits unlock AI form generation and regeneration in the app. Failed or schema-invalid generation does not consume credits.
• Credits are bought in the App Store or Google Play — not on this site.
• Existing forms remain editable and publishable at zero credits; only generation and regeneration are blocked when credit balance is insufficient.

Refunds

Store refund events do not reverse Formtasia credits. Requesting a platform refund does not restore credits inside Formtasia.

Exports

Creators and editors with permission may export responses to Excel from the mobile app. Private viewers cannot export. Exports produce time-limited download URLs handled by the backend.

Acceptable use

Do not use Formtasia to collect unlawful data, harass respondents, or abuse rate limits. We may suspend accounts that violate these product rules or applicable law.
